Making holes in layer mask?

I've got CS2, and am following a tutorial in Issue 59 of Photoshop Creative
magazine. I'm sort-of familiar with Photoshop's basics, and this one should
be as basic as it comes, but I can't figure out what I'm doing wrong.

The subject file is a JPEG image. The image layer was double-clicked and
turned into a layer from its original Background state.

The tutorial tells you to use the Pen tool to make a Path around the part
of the image you want to keep. Then right-click, and choose "Make a
Selection" from the drop-down menu. Then click the Add Layer Mask button at
the bottom of the Layers palette. So far, so good: The image disappears
except for the bit inside the Mask, exactly the desired result.

But here's where the problem comes in: Now I want to add holes that are
internal to the Mask. Specifically, the image is a truck, and the space
between the side mirror and the windshield needs to be empty, so that a new
background will be visible through there.

The tutorial has you selecting the Mask thumbnail in the Layers palette,
and then using the Pen tool to pick out the hole you want, directly on the
image. Once your hole is defined, you then right-click, and "Make a
Selection" again. Then you hit the backspace key to make the hole visible.

Everything is fine right up to the Backspace key part. I never get the new
hole to show up.

What am I doing wrong?

Update:
I found my mistake. Or maybe it's not a mistake, but some setting I've
changed inadvertently. I can get the holes to show up if, after hitting
Backspace, I also hit Ctrl+I to invert. And lo, the hole reveals itself.
